Why hand carry suits pharma work and where it stops

Pharma and clinical consignments fail in the gaps between handovers, not in the air. A box that sits on an apron in the sun, waits overnight in an uncontrolled warehouse or is held in a customs shed for two days has usually spent longer outside its range than the flight took. A hand carry removes those gaps: one person collects the box, keeps it with them and hands it to a named person at the other end. For a shipment whose value is a study timeline, that is often the whole argument.

Good distribution practice rests on showing what happened to a product, and a hand carry produces an unusually clean record: one custodian, a logger that ran continuously, timed photographs at collection and handover, and a signature. Where an excursion occurs, the record shows exactly when, which is what lets a quality team make a release decision instead of writing the consignment off.

What temperature controlled means on a hand carry

The shipper names the range, commonly refrigerated at two to eight degrees Celsius, controlled room temperature, frozen, or deep frozen on dry ice. The range is part of the product's licensed or protocol defined storage condition, and it comes with an excursion budget the quality function has already calculated. The courier is briefed on the range, the duration the box is validated for, and what to do if either looks at risk.

Nothing in an aircraft cabin is temperature controlled the way a pharma warehouse is. The cabin is comfortable for people, the hold is not controlled in the sense the product needs, and an apron in a hot climate is worse than both. Everything depends on the passive box and the time it is exposed. The plan is built backwards from the validated duration, so the box is opened before its window closes rather than after.

Validated shippers and pre conditioning

The shipper provides the box and conditions it. A passive validated shipper works because its phase change material has been brought to a specified temperature for a specified time before packing, and because the payload is packed as the validation report says. A box packed in a hurry with warm bricks, or with the payload pressed against the coolant, will not perform as the report claims. The courier collects a box already prepared, not components to assemble.

Two practical points at collection. The validated duration starts when the box was closed, not when the courier arrived, so that time is recorded and photographed. And the box is not opened in transit for any reason, including inspection, without the shipper being contacted first. If security or customs insist on opening it, the courier reports it with a time, so the quality function can assess the exposure rather than guess at it.

Data loggers and the temperature record

A logger travels with the payload and, on many consignments, a second logger sits on the outside of the box so ambient conditions are recorded too. Single use loggers with a printable file are the norm for study samples, and connected loggers that report in real time are used where a quality team wants to see an excursion while it is happening. The shipper decides which, activates it and records the activation time.

The record only helps if it is complete and reaches the right people. The logger is started before packing and stopped by the receiving site, not by the courier, and timed photographs at collection, handover and delivery fill the gaps between logger events. Where a real time device raises an alert in transit, the courier calls the controller and the controller calls the shipper. Nobody makes a product decision in an airport concourse.

Clinical trial samples and investigational product

Study work has its own blockers. Samples move from a site to a central laboratory under a protocol naming the courier arrangements, the temperature range and the maximum time in transit. Investigational product moves the other way, from a depot to a site, sometimes with a randomisation code that must not be broken and packaging that must not be relabelled. Both need the study number, the site number and the receiving contact on the paperwork.

Most biological samples travel as a category B biological substance, in the triple packaging that classification requires, with an absorbent layer, a rigid outer and the correct marking. The packing is the shipper's responsibility and the courier checks that the outer marking matches what has been declared. Category A material does not travel with a passenger. If nobody on the file can say which category applies, that is answered before anything is booked.

Paperwork on arrival

Pharma consignments meet more agencies at the border than industrial goods do. Alongside the customs entry there may be a health authority import permit, a licence for the specific product, a certificate of free sale, evidence of good manufacturing practice status, or a controlled substance licence held by the importer and matched at origin. Some countries want the permit reference on the invoice itself, and none accept an application made after the goods have landed.

Mexico is a useful example because the rules are explicit. Imported medical devices and health products need a sanitary import permit from the health regulator before release, labelling has to satisfy the applicable Mexican official standard, and the entry still needs a pedimento filed by the consignee's broker against an active importer registry entry. Those are separate approvals and both must be in place. Ask for the permit number before the courier leaves.

What cannot travel in a cabin

Some consignments are outside the mode. Category A infectious substances cannot travel as passenger baggage. Radioactive material cannot either, which rules out most radiopharmaceutical work and sends it to a dedicated cargo route. Liquid nitrogen in liquid form is not carried by a passenger, although a dry vapour shipper holding no free liquid is treated differently and may be acceptable subject to the airline's approval.

Dry ice is the one that catches people out. It is a dangerous good, a passenger may carry only a limited quantity and only with the operator's approval, and that quantity is confirmed with the specific airline before a seat is bought. Flammable solvents, some reagents and pressurised cylinders are refused outright. A hand carry suits refrigerated and controlled room temperature work well, frozen work sometimes, and deep frozen work only with the airline's written agreement.

02Can the courier carry dry ice?

Only a limited quantity, only with the operating airline's approval and only when it is declared correctly. The permitted quantity is confirmed with the specific airline before a seat is bought, so send the dry ice weight with the enquiry.

03Will the box be opened by customs or security?

It can be. If that happens the courier reports it immediately with a time and a photograph rather than resealing it quietly, so the quality function can assess the exposure against the excursion budget instead of finding it in a logger file days later.

04Can you carry clinical samples across a border?

Yes, where they are packed and classified as a category B biological substance and the receiving laboratory is expecting them. Category A material does not travel with a passenger. The study number, the site number and the laboratory contact go on the paperwork.

05What about a controlled drug?

That needs a licence held by the importer and normally a matching export licence at origin, both in place before departure and both referenced on the documents. Without them the consignment does not move, and urgency changes nothing.

06Who signs for the delivery?

A named person at the laboratory, pharmacy, depot or hospital receiving department, within their own opening hours. Those hours are agreed at booking, and where the deadline falls outside them the routing is built around the opening time.
